SFI (AKA Stupid F'ing Infection)

So I got the pathology report and requisition for the thrombophillia panel in the mail yesterday. WTG nurse D for getting that out so promptly and to the postal service for record overnight delivery!

I was reading through the pathology last night. It's not easy to understand. I am not a physician and do not use or know many of the terms that have been used.

This was the"Final Diagnosis"
Placenta:
Immature Placenta
Placental weight < 25th percentile for gestational age
Acute Chorioamnionitis, fetal membranes and chorial plate
Chronic Basal Deciduitis
Placenta Hematoma, Recent

After I read through I was so mad. That SFI! Even though Dr. T said there was no way of knowing when or how the infection appeared it just makes me mad that it was even there in the first place. Chorioamnionitis is so rare according to information I have read on the internet it occurs in 1-2% of all pregnancies in the US and most often occurs after a prolonged labour.

Elsewhere in the microscopic description it says the fetal membranes "demonstrate patchy acute chorio" and the chorial plate "demonstrates early chorio" Oh yes Deciduitis is also infection.
Unfortunately it is possible to get this SFI even with intact membranes. So I guess the infection was "new" but if it was the cause of PTL or a result of PTL or IC I will never know. My biggest concern is if the SFI is still there hiding :( I hope the 4 bags of IV antibiotics sufficiently squashed any leftover infection.

I did notice that in terms of the SFI the placenta was birthed at oh 3am on the 9th, received on the 11th and reported on the 12th in the afternoon.

The "recent" retroplacental hematoma was HUGE 164g compared to the 120g placenta. It says the maternal floor of the placenta was 90% fragmented and the most fragmented margin was where some of the hematoma was attached. Anyways this could mean an abruption in the true sense however I have learned that acute chorio SFI can rupture the "spiral arteries" which then causes the placenta to separate.

I do find it interesting that the placenta was <25th percentile for gestational age. There are other comments like "immature" and "poorly developed vascualture" But I have to wonder if that was just because I was only 21 weeks. It's not like Gavin was small for gestational age or anything. He was a good weight and normal length.

I am miserable.....had an awful evening, awful night....stupid cold.

My name is Landon. I have no answers for you. I am following your blog. I cannot fathom the depth of your pain. My heart truly goes out to you! I am glad that you are writing about your grief and you are grieving at your own pace. That, alone, is progress, I believe.
There are some things I do know. Miscarriage mortality percentages are tragically high and our technology is still sadly lacking. It has years of development to go, possibly decades of lab research before they can give answers to the big WHY questions. They really don't know why all miscarriages happen, it is beyond horrific when this terrible tragedies happen, beyond horrific. I am soo sad that this happened to you.
But, I am grateful that you are grieving at your own pace. You aren't stuck.
